As a part of the Children Provider Network, PCN along with Chicanos Por La Causa, Native American Connections, Empact, Ebony House, and Centro de Amistad work in collaboration with your child or youth, family and others who are important to deliver culturally attune and comprehensive behavioral health services to children, youth and their families living in Maricopa County

Aligning with the Arizona Model

In collaboration with the child, family and other collaborative partners, PCN's – Children Provider Network will provide accessible behavioral health services uniquely designed to assist children to achieve success in school, live with their families, avoid delinquency and become stable and productive adults. PCN and our providers offer services tailored to each child and family, delivered in the most appropriate setting, in a timely fashion and in accordance with best practices, while respecting the child and family’s cultural heritage

1) Collaborations with Children and Families - Collaboration is essential during every stage of service delivery with children and their families. We are on the team together – partners with children and parents.

2) Outcomes –Our behavioral health services assist children to be successful in school, return to live with their families, avoid delinquency, live stable lives to become productive adults.

3) Child/Family Centered - When children we serve have multi-system involvement, we will have jointly established centered service plans..

4) Access Children will have access to a wide array of behavioral health services, which we will seek to adapt or create when not available.

5) Best Practices – Our services are provided according to best practices, evaluated and modified to achieve desired outcomes for children and families.

6) Convenient & Comfortable Settings – Children receive services in their homes and community to the extent possible.

7) Timely ServicesChildren identified as needing behavioral health services are assessed and served in a timely manner.

9) Maximize Stability - Our behavioral health services strive to minimize multiple placements and prevent crisis situations.

10) Cultural Traditions – Our network providers provides services for people from diverse communities in a respectful manner that honors and understands the cultural tradition of the children and families we serve.

11) Gain Independence – Our behavioral health services include support and training for both parents and children to gain independence.

12) Natural Supports – Our services encourage families to discover, develop and connect to their own community “natural supports” available from the child and parents’ own network including friends, neighbors, schools, community and faith-based organizations.
