Case Management

Intensive Case Management for High Complexity/High Intensity Children is an opportunity to partner with the provider staff in addressing challenging issues such as identifying community and system resources, connecting, collaborating and coordinating care with System partners and others and supporting the work and effort of the team to meet Individual Service Plan goals and objectives.

POCN children's services case managementGoal: “A resource to support the Child and Family Team in overcoming and problem solving difficult challenges, identifying additional resources, and gaining a sense of incremental movement toward achieving care goals.”

Method: “consulting, mentoring, coaching”

Consulting:

  • What is being done, why is it being done, what are the results?
  • “What’s working, what’s not working and Why?”
  • “What evidence based interventions have we tried or could try if we haven’t?”
  • “What is best practice?”
  • “What are system requirements?”
  • “What outcomes do we hope to achieve?”
  • “What if any assistance does the team need?”

Mentoring: Defining, clarifying:

  • How is the team functioning?
  • What are its strengths?
  • What are its possibilities?
  • What are the biggest challenges facing the team?
  • Is the Child/youth engaged?
  • Is the family engaged?
  • Does the family understand its role?
  • What natural and community supports are working?
  • What is the satisfaction level of the child and family?
  • What System resources are available and how and who will make those connections?

Coaching: Supporting, modeling, teaching, showing, advocating

  • Engage family and child in care
  • Help “tease out” what the natural and community supports are
  • Help identify and make connections with natural and community supports and help with transitions
  • Follow up and follow thru with family and supports
  • Feedback to team/child/youth/ family
