A retrospective analysis published in the Journal of Clinical Oncology suggests that patients with node-positive nonmetastatic bladder cancer should have the option to receive bladder-sparing trimodal therapy as an alternative to radical cystectomy. The study showed that patients treated with radical therapy had significantly better overall survival compared with those receiving palliative treatment. Moreover, the type of radical treatment (RadRT or radical cystectomy) did not significantly impact survival outcomes.

The authors suggest that patients should be counseled about their treatment options, and bladder preservation should be considered as a real alternative to radical surgery, even in the presence of pelvic lymphadenopathy.
