Una comunidad de agencias de salud comportamental que atienden a niños y jóvenes de familias residentes en diversas comunidades, proporcionando tratamiento y rehabilitación domiciliarios y comunitarios de manera respetuosa, accesible y sustentable.
Adult Services
At PCN’s three neighborhood sites we serve adults with serious mental illness living in Maricopa County. Our staff and community service providers are dedicated to serving and supporting adults in the context of their family, community, and those significantly involved in a manner that is respectful and culturally responsive. Together, we can help to build on an individual’s strengths – to better access and make use of community resources, case management, and psychiatric services.
NEW Integrated Health Home Program (IHH) available for our adult participants at our Comunidad site and opening soon at the Capitol Center site in partnership along with Magellan Health Services of Arizona and Maricopa Integrated Health System (MIHS) with primary care professionals as part of an co-located integrated care team to improve the quality of life and increasing life expectancy for people experiencing serious mental illness, behavioral health challenges and co-occurring substance abuse disorders focusing on whole health (mind, body + spirit);
Children's Services
PCN understands that reaching out for help for a child or family member is not easy. With our culturally attune and respectful practice-based child-family team approach, we are here to give support and hope. Conveniently located in your neighborhood, our network offers an impressive group of behavioral health agencies with a successful history of serving children, youth and families living in Maricopa County.
